Abstract

Background: Maternal and child health is an essential indicator in measuring the health status of a country. Every day, 830 women die due to pregnancy and childbirth. Of the 189 million women who became pregnant, 122 million had live births and nearly 3 million suffered stillbirths. Various efforts have been made to reduce Maternal Mortality Rate (MMR), including quality and sustainable health services starting from pregnancy, antenatal care or prenatal surveillance to obtain information on maternal and fetal health, early detection of diseases accompanying pregnancy, and normal delivery preparation.
Case Report: Comprehensive midwifery care was performed on Mrs H. The data collection techniques were anamnesis, interview, observation, and examination. The data analysis compared the data found and the existing theory.
Discussion: This case report details the continuity of care for Mrs. H, a third trimester patient of pregancy, using the SOAP method
Conclusion: Complete continuity of care has been wholly and procedurally using the SOAP method. No gap was found in the subjective and objective data. In other words, the analysis and the management have been under the theory
