Abstract

Background: Data from the World Health Organization (WHO) shows that the world's maternal mortality rate (MMR) due to complications from pregnancy or childbirth is estimated at 8.30 per 100,000 live births. Family health program records at the Ministry of Health in 2019 show an increase in MMR from 4,221 cases in 2019 to 4,627 in 2020. The cause of maternal death was bleeding (5.13 cases or 130/100,000 live births). Therefore, the government seeks to reduce MMR and IMR through a comprehensive midwifery care program that includes integrated midwifery care programs starting from pregnancy, childbirth, postpartum, and newborns to family planning. In this case, the program is conducted using a sustainable midwifery care model and the soap method.
Case Report: Comprehensive midwifery care was performed on Mrs. A (23 years, G2P1A0 40) and her baby from September 12, 2022 to February 13, 2023 at the Pal IX Health Center of KubuRaya regency. The type of the data was primary. The data collecting techniques were anamneses, examination, observation, and documentation. The data, then, were analyzed by comparing the data gathered and the existing theory.
Discussion: This case report details the continuity of care on a patient with second and third pregnancy, using the Soap method.
Conclusion: Complete continuity of care has been wholly and procedurally using the SOAP method. A gap was found in the subjective and objective data of Mrs A and her baby, particularly during childbirth, postpartum, new born, and vaccination
