Abstract
Background: According to the most recent data from World Health Organization (WHO), every day in 2017, approximately 817 women died from preventable causes related to pregnancy and childbirth. In 2020, 4,627 maternal deaths in Indonesia were recorded (Ministry of Health Republic of Indonesia, 2021). Meanwhile, in 2019, as many as 117 cases of maternal deaths took place in West Kalimantan (West Kalimantan Provincial Health Office).
The period of pregnancy, childbirth, postpartum, and neonatal are a physiological condition that may threaten the life of the mother and the baby. Therefore, a Continuity of Care (CoC) needs to be carried out as an early detection of high risk maternal and neonatal mortality (Yulia et al, 2019).
Case Report: : A continuity of care was performed to Mrs. N at a private midwife clinic ‘Ida Apianti’ Pontianak from November 11, 2021 - December 21, 2021. The subject was Mrs. N (35), G3P2A0, 38 weeks pregnant. The types of the data were primary and secondary. The data collecting method were anamnesis , observation, examination and documentation. The data was analyzed by comparing the data obtained with the existing theory.
Discussion: This case report details a continuity care to a pregnant woman, from the third trimester of pregnancy until the third visit of postpartum, using the SOAP method.
Conclusion: The continuity of care has been performed using a SOAP documentation approach. No complications of pregnancy, childbirth, postpartum, neonates found during the care.
